State Probate Forms
for the State of New York

All of the New York State forms are available at:

www.nycourts.gov/forms/surrogates/probate.shtml

These forms should be saved to your computer and can be filled out after saving and subsequently printed. Electronic submission of these forms is not possible.

The easiest way of filling out these forms and maintaining the integrity of the form when printing out again is to use Adobe Acrobat and select those versions of the form.
